Special Practice Notes
Personal Health Information
This practice is committed to maintaining the confidentiality of your personal health information. Your medical record is a confidential document and is treated as such. As is the policy of this practice to protect your personal health information, we ensure that this information is only available to authorised staff members for the intended purposes and to comply with Privacy Act.
Most problems are best dealt with in consultation. A doctor is available during normal surgery hours for emergency advice. Our staff are experienced in deciding the appropriate response to any phone request.
Because our practice is committed to preventative care, we may send you an occasional reminder regarding health services appropriate to your care. If you wish to opt out of this, please let us know.
To ensure continuity of care, this practice does not release test results on the phone (To discuss the results please make a follow-up appointment)
This practice does not honour repeat prescription telephonic requests unless in exceptional circumstances. Attendance is necessary. A token fee of $15 per script or $20 for 2 scripts is charged.
This may be arranged if too unwell (but not urgent) to come to the surgery, for Mackay local residents and at no inconvenience to booked practice patients.
Please feel free to talk to your Doctor or the Office Manager about any problems you have with the service we provide. We appreciate your comment and feedback as we strive to improve for your benefit. If you wish to take any matter further, you may contact Health Rights Commission, GPO Box 3089, Brisbane 4001.
